What is Orthorexia Nervosa?
Orthorexia Nervosa is a disordered eating pattern that comes with an obsessive drive to eat “right.” It is characterized by rigid rules around food and eating. Most often, the focus is on healthy eating (clean eating), and when someone cannot follow the rules, they experience significant distress. At this time, it is not included in the DSM-5 (DIAGNOSTIC AND STATISTICAL MANUAL OF MENTAL DISORDERS). Specialists believe that it will be included in its next iteration.
Orthorexia differs from anorexia because the focus of the anxiety is on food quality or rules, not someone’s weight or physical appearance. Orthorexia is a serious condition and can result in medical and psychological complications.
How Does Someone Develop Orthorexia?
For many people, orthorexia begins with what feels like reasonable health awareness — preventing illness, managing a condition, or simply wanting to feel better. The problem isn’t the intention. It’s the slow narrowing of what feels “safe” to eat, the tightening grip of food rules, and the rising distress when those rules are challenged. That shift — from healthy interest to rigid obsession — is where clinical concern begins.
If any of this resonates with you or someone you care about, you don’t have to sort through it alone. Our dietitians at nycnutritionist.net specialize in helping folx untangle disordered eating from genuine health — without judgment and without losing what matters to you.
We provide you with evidence-based nutrition therapy. Together, we will work to help you overcome this drive for perfection and improve your relationship with food.

Who is at risk for developing orthorexia?
Orthorexia nervosa doesn’t discriminate by age, gender, or background — but certain factors do increase vulnerability. Research points to a mix of psychological traits and cultural pressures that create fertile ground for the disorder to take root.
Risk Factors
- Perfectionism and rigid self-standards
- A strong need for control
- Difficulty regulating emotions
- OCD or significant obsessive-compulsive features
- A personal or family history of disordered eating or an eating disorder
- Heavy exposure to diet culture and “healthism” content — particularly wellness influencers, clean eating accounts, and messaging that frames food in moral terms (good/bad, clean/dirty, pure/impure)
What is “healthism culture”?
We are consumed by diet culture, a culture that equates thinness and clean eating with moral virtue. We’re sold the idea that everything (including our weight and body shape) is under our control.
Healthism takes diet culture to an extreme, framing wellness as each person’s total responsibility. How we eat and our wellness is within our complete control. There are influencers and billionaires who amplify this message. This cultural context plays into orthorexia nervosa and people’s vulnerabilities to developing it.

What does orthorexia treatment look like with NYC Nutritionists?
Recovery from orthorexia isn’t about giving up on health — it’s about reclaiming it. Our Registered Dietitians at nycnutritionist.net take a weight-inclusive, non-diet approach that honors your desire to take care of your body without letting that care consume your life.
We help by:
- Rebuilding flexibility. We’ll work with you to reintroduce previously restricted foods gradually, so eating feels less like a test and more like nourishment.
- Challenging food rules. Together, we identify the rigid beliefs driving your choices — and replace them with balanced, evidence-based guidance.
- Separating health from morality. You’ll learn to see food as neutral: no “good” or “bad,” no “clean” or “dirty.” Just food.
- Reducing food anxiety. Through meal-planning support and exposure work, we help you feel safe eating outside your comfort zone.
- Addressing the root drivers. We explore the perfectionism, control needs, or cultural pressures that fuel orthorexia — and give you better tools to handle them.
- Coordinating with your team. We work with your therapist, doctor, or other providers to ensure your recovery is supported from every angle.
Our goal: Help you eat well without it taking over your life. You don’t have to choose between your health and your freedom. At nycnutritionist.net, we help you keep both.
Signs & Symptoms of Orthorexia
Orthorexia isn’t about what you eat — it’s about how you think about food. Unlike other eating disorders, orthorexia may not center on weight or body image. Instead, it’s driven by an obsession with purity, health, and control. Here’s what to look for.
Behavioral Signs
- Rigidly avoiding foods deemed “unhealthy,” “impure,” or “processed”
- Spending excessive time researching, planning, and preparing “clean” meals
- Cutting out entire food groups (grains, dairy, sugar, etc.) without a medical reason
- Increasingly narrowing the list of “acceptable” foods over time
- Bringing your own food to social events or refusing to eat outside the home
- Feeling anxious or distressed when “safe” foods aren’t available
Psychological & Emotional Signs
- Intense guilt or shame after eating something outside your food rules
- Feeling superior to others based on what you eat
- Defining self-worth through dietary choices
- Difficulty relaxing or enjoying food — meals feel like a moral test
- Preoccupation with food that interferes with work, school, or relationships
Physical Signs
- Unintended weight loss or nutritional deficiencies
- Digestive issues (bloating, irregularity) from an overly restricted diet
- Fatigue, low energy, or declining physical performance — despite believing you’re eating “healthy”
- Loss of menstrual cycle (in menstruating individuals)
Social Signs
- Avoiding restaurants, family dinners, or social gatherings involving food
- Strained relationships because of your food rules or judgments about what others eat
- Spending less time with friends and more time on meal prep and planning

What If I've Tried Other Treatment and It Didn't Work?
Not all treatments work for all people. And because you haven’t been successful, that doesn’t mean every treatment plan will be the same. Orthorexia can be isolating, and we want to help you. Treating disordered eating behaviors is complex, and each patient is unique. A successful treatment plan:
- Needs to be evidence-based.
- Must prepare you for real-world triggers, giving you the coping skills to manage everything from restaurants and family meals to a stressful day.
- Must have a team that is resourceful in helping you navigate setbacks. There is no direct route to healing, and setbacks are going to happen.
- Includes your support network – spouses, parents, friends, and close loved ones. It takes a community to work through this.
- Prioritizes your health and well-being to ensure you are getting the nutrients you need.
Will You Coordinate with My Therapist and Doctor?
Absolutely. The most successful treatment for orthorexia involves coordinated care with your therapist, physician, and support team. Together, we will work to build a plan to help you manage mealtime stress.
Can you recover from orthorexia?
Yes, you can.
There is no magic timeline for recovery. For some patients, it might take months. Others can take years. Setbacks and relapses tend to be the rule, not the exception. With the right team and support, however, you can overcome orthorexia and live a healthy life.
Do You Accept Insurance?
Absolutely! 95% of our patients pay little to nothing out-of-pocket. We also offer competitive private-pay rates for patients without insurance.
We will check your coverage for you and pursue authorization when appropriate.
