What is OSFED?
Someone with OSFED (Other Specified Feeding or Eating Disorder) does not meet the criteria to be diagnosed with a specific eating disorder; however, they present many symptoms of other eating disorders. The DSM-5 uses OSFED to diagnose people who have serious eating problems.
OSFED is a serious condition and can result in medical and psychological complications.
What are examples or subtypes of OSFED?
- Subthreshold bulimia: Bingeing and purging occur, but less frequently or for a shorter duration than the full diagnosis requires.
- Subthreshold binge eating disorder: Bingeing happens, but not as often or for as long as the standard criteria specify.
- Purging disorder: Purging (self-induced vomiting, laxatives, etc.) occurs without binge eating episodes.
- Night eating syndrome: Consuming a significant amount of food after waking during the night, or eating excessively after the evening meal.
- Weight suppression without clinical “underweight”: Significant weight loss is achieved, but the body remains in or above the “normal” BMI range — which can still carry serious metabolic, hormonal, and psychological effects.

Who is at risk for developing orthorexia?
Orthorexia nervosa doesn’t discriminate by age, gender, or background — but certain factors do increase vulnerability. Research points to a mix of psychological traits and cultural pressures that create fertile ground for the disorder to take root.
Risk Factors
- Perfectionism and rigid self-standards
- A strong need for control
- Difficulty regulating emotions
- OCD or significant obsessive-compulsive features
- A personal or family history of disordered eating or an eating disorder
- Heavy exposure to diet culture and “healthism” content — particularly wellness influencers, clean eating accounts, and messaging that frames food in moral terms (good/bad, clean/dirty, pure/impure)
What is “healthism culture”?
We are consumed by diet culture, a culture that equates thinness and clean eating with moral virtue. We’re sold the idea that everything (including our weight and body shape) is under our control.
Healthism takes diet culture to an extreme, framing wellness as each person’s total responsibility. How we eat and our wellness is within our complete control. There are influencers and billionaires who amplify this message. This cultural context plays into orthorexia nervosa and people’s vulnerabilities to developing it.

Signs & Symptoms of Orthorexia
Orthorexia isn’t about what you eat — it’s about how you think about food. Unlike other eating disorders, orthorexia may not center on weight or body image. Instead, it’s driven by an obsession with purity, health, and control. Here’s what to look for.
Behavioral Signs
- Rigidly avoiding foods deemed “unhealthy,” “impure,” or “processed”
- Spending excessive time researching, planning, and preparing “clean” meals
- Cutting out entire food groups (grains, dairy, sugar, etc.) without a medical reason
- Increasingly narrowing the list of “acceptable” foods over time
- Bringing your own food to social events or refusing to eat outside the home
- Feeling anxious or distressed when “safe” foods aren’t available
